Özet

In our study we aimed to determine the effects and safety of intranasal midazolam, ketamine, alfentanil and fentanyl on children premedication. Seventy-five children, 2-7 years old were included in our study and they were randomly allocated to five groups. Twenty minutes before the operation serum physiologic 2 cc, midazolam 0.3 mg/kg, ketamine 5 mg/kg, alfentanil 30 μg/kg and fentanyl 10 μg/kg intranasally were administered in Group 1,2,3,4 and 5; respectively. After induction of anesthesia with thiopentone 4-7 mg/kg and vecuronium 0.1 mg/kg IV anesthesia was maintained with isoflurane 1% and 50% nitrous oxide in oxygen in all the groups. After monitoring the patients noninvasively systolic, diastolic and mean arterial pressures, heart rate, respiratory rate, oxygen saturation and sedation scores were recorded just before the premedication and for 20 minutes after the premedication at an interval of 2.5 minutes. Children's leaving the mother and response to intra-venous cannulation, pre and postoperative complications were also evaluated. As a result we concluded that midazolam and alfentanil, with their early sedation and low postoperative complication rates, can be preferred to ketamine and fentanyl but with its early recovery alfentanil seems to be superior.
