Özet

Diagnosis and staging of lung cancer should be managed promptly and accurately by minimizing procedures efficiently. Within the multidisciplinary team approach, minimally invasive bronchoscopic and transthoracic procedures provide rapid and safe tissue acquisition. Ideal tissue acquisition depends on the individual patient and need for sufficient tissue for pathologic, immunohistochemical and molecular studies. Combining diagnostic and/or staging techniques strategically provides better yields and management outcomes. For pathologic, immunohistochemical and molecular studies, specimens should be sufficient in quality and quantity. Immunohistochemical studies should be performed judiciously to preserve cellular material for downstream molecular testing. In the near future, multiplexed panels (e.g. next generation sequencing) will be a must for a panel beyond three biomarkers: EGFR, ALK and ROS1. Rapid on-site evaluation, sensitive genotying assays and/or liquid biopsy can be used to overcome challenges of inadequate tumor tissue, tumor heterogeneity, heterogeneous resistance mechanisms, and poor performance status.
