Intravenous clonidine for postanaesthetic shivering complication

Özet

Postanaesthetic shivering is a common complication. It is related to hypothermia and spinal reflexes unopposed by a still anaesthetized brain. In our study, we investigated the effect of an alpha-2 agonist, clonidine on postanaesthetic shivering. In our study 16 patients (ASA I-II) who received therapy for postanaesthetic shivering were randomly allocated to two groups. In the first group (placebo group) (n = 8) treatment began with 5 cc saline IV, followed by 150 microgram clonidine 10 min. later, if the initial therapy showed no improvement. The second group (clonidine group) (n = 8) was initially treated with 150 microgram clonidine IV, followed by 5 cc saline IV 10 min later, if the initial treatment failed. Treatment of shivering was evaluated with 3 point scale (0: no improvement, I: partial improvement, II: complete improvement). Arterial pressure, heart rate, oxygen saturation were monitorized. Data were statistically evaluated with paired-t test. In group I (placebo group), none of the patients improved and required a second injection. All the patients in group II improved following the first injection (p < 0.05). The onset of action was 180 ± 7.2 sec. after injection of clonidine. No significant changes occurred in hemodynamic data. Analgesia and sedation were observed in all patients of clonidine group. In conclusion, our study shows that IV clonidine after a comfortable postoperative period inducing analgesia and sedation and is useful for treatment of postoperative shivering.
