Unilateral Adrenal Vein Sampling and Contralateral Suppression Index are Good Indicators for Adrenelectomy in Conn's Syndrome

Adrenal vein sampling is a technically challenging procedure, successful cannulation rate of both adrenal veins was reported as 95.6% in experienced hands and drops to 30–50% in other series. When inability to cannulate one of the adrenal vein, adrenal vein/inferior vena cava index can highly predict unilateral disease. In this study, our single center adrenal vein sampling results between 2020 and 2022 were examined and the postoperative data of patients who were one-sided catheterized and operated according to their contralateral suppression index results analyzed. Adrenal vein sampling was applied to 21 patients between 2020 and 2022, Ten of the adrenal vein samplings were unilateral catheterized and sixty three percent of the bilaterally catheterized patients had lateralization. All ten patients with unilateral characterization had contralateral suppression index < 0.5 and half of them had contralateral adrenal adenoma. Contralateral adrenalectomy was performed in ten patients according to contralateral suppression index results, 90% cured after operation. Remission was achieved with contralateral adrenalectomy in 90% of patients with contralateral suppression index < 0.5. In order to make an operation decision according to the contralateral suppression index criteria, a larger patient cohort with real-life data is needed. Although our study is small, it is valuable because it contains real-life data.
