Aim: In our study, correlation between serum alanine aminotransferase (ALT), histological activity index (HAI), HBeAg/antiHBe seroconversion and HBV-DNA levels in chronic hepatitisB (CHB) patients with higher ALT levels was investigated. Material and Method: Liver biopsy was performed on 31 untreated patients with CHB. Tissue HBV-DNA was measured from 19 samples from slides following histopathological examination and from the rest 12 from direct biopsy materials. Serum HBV-DNA was also measured for all patients. Hepatitis B serological pattern, Knodell HAI and ALT levels of the patients were analyzed and recorded. Results: Serum HBV-DNA levels of HBeAg(+) patients were found significantly higher than those in patients with HBeAg(-) (p<0.01). Although a significant difference was found in ALT levels of patients with HBeAg (+) and (-) (p=0.02), Knodell HAI data showed no correlation (p=0.44). No correlation was also observed between ALT and Knodell HAI (p=0.18). Serum HBV-DNA and ALT levels and Knodell HAI didn't indicate any significant correlation between each other (p=0.06 and p=0.46, respectively). Four slide samples and 9 biopsy materials had HBV-DNA(+). Discussion: In CHB patients with increased ALT and HBeAg(+), serum HBV DNA levels can be ≥105 copy/ml. Additionally, serum HBV-DNA(+) might be obtained in patients with seroconversion of HBeAg/antiHBe. Those patients might also had liver HAI which isn't correlated with HBV-DNA and ALT levels. So, we may conclude that serum HBV-DNA levels associated with histopathology and ALT may be used as a guide for clinical prognosis when it is evaluated with liver.
