Özet
Closed-loop ventilation modes adjust automatically certain ventilators settings in order to keep physiological variables in target ranges set by the users. Continuous adaptation of the ventilatory support to the changes in the patient’s respiratory mechanics, spontaneous activity, and effort result in more time spent in optimal ventilation and oxygenation ranges, and adequate ventilation support that may reduce the risk of ventilator-induced lung injuries, patient-ventilator asynchrony, and over- or under-assistance. Ultimately, clinical outcomes-such as duration of mechanical ventilation and mortality-may be improved. This chapter details the principle of operation, initial settings and adjustments, monitoring, and evidence of the closed-loop modes commercially available, namely Mandatory Minute Ventilation (MMV), Smartcare/PS, Adaptive Support Ventilation (ASV), and INTELLiVENT-ASV.
