Özet
Intravenous administration of high dose immunoglobulin (Ig) may increase immune defence by providing large amounts of antibodies. The purpose of this prospective, randomized, placebo-controlled study was to evaluate the effectiveness if IgG on mortality for septic, high risk surgical patients undergoing intensive care treatment. Forty septic patients receiving intensive care treatment were randomly taken into two groups at Gulhane Military Medical Academy General Surgery Department. Both groups were similar in initial sepsis and APACHE II scores, sepsis score 16 or higher. Study group of 21 patients received high dose IV immunoglobulin, so Human Albumin was administrated for control group of 19 patients. At the end of the study, significantly lower mortality was recorded in IgG treated group (14%) than in control group (47%). None of the patients in IgG treated group did not die of septic shock but septic shock was the cause of death 26% of patients in control group. Almost identical proportion of IgG and control patients died of Multiple organ failure (14% to 15%). The results of this study indicate that high-dose IgG improves survival and decreases death from septic shock in surgical intensive care patients.
