Özet
Immunoglobulin-A (IgA) deficiency is a common primary B-cell immunodeficiency. Although most of the patients with IgA deficiency are healthy, some patients may have recurrent sinopulmonary infections, allergic disorders and autoimmune diseases. It is also well known that the patients with selective IgA deficiency have an increased prevalence of rheumatic diseases. The incidence of IgA deficiency varies from 1/400 up to 1/3080 in Turkish population. However, this may be encountered more frequently in patients with systemic lupus erythematosus and rheumatoid arthritis (1/32 and 1/25, respectively). We report a 21-year-old case with selective IgA deficiency and positive rheumatoid factor (RF) complicated with renal involvement. He was admitted to our clinic with fever, cough and ulcers on his tongue, later detected to have microscopic hematuria, proteinuria and Coombs' positive haemolytic anaemia. This case is presented on account of its rare association with selective IgA deficiency such as autoimmunity and renal complication. In selective IgA deficient subjects, early diagnosis of renal and autoimmune diseases is crucial for the early management of probable complications. Thus, we recommend close follow up of these patients with selective IgA deficiency closely to avoid unwanted further complications.
