Evaluation of Perinatal and Neonatal Outcomes in Pregnant Women with Thyroid Autoantibody Positivity (Anti-thyroglobulin and Anti-thyroid Peroxidase) Due to Thyroiditis

Özet

The aim of this study was to evaluate the maternal–fetal and neonatal outcomes in cases with thyroid autoantibody (anti-thyroglobulin [TG] and anti-thyroid peroxidase [TPO]) determined as positive during pregnancy. The study included 46 pregnant women (n = 11 and n = 35, respectively) with high results in thyroid autoantibody screening. The groups were compared in respect of demographic data, antenatal screening test parameters, clinical characteristics, gestational diabetes mellitus (GDM) screening tests, complications during pregnancy, and perinatal and neonatal results. No statistical difference was determined between the groups in respect of the antenatal screening tests of combined test beta-human chorionic gonadotropin (β-hCG), pregnancy-associated plasma protein-A (PAPP-A), nuchal translucency (NT) multiples of median (MoM) and triple/quadruple screening test hCG, unconjugated estriol (uE3), and alpha-fetoprotein (AFP) MoM values (p > 0.05). A positive 50 g oral glucose challenge test (≥ 140 mg/dl) was recorded in 25% of the anti-TG group and in 21% of the anti-TPO group, with no statistical difference between the groups (p = 0.542). GDM was present in 10% and 14.2%, respectively, of the two groups with no statistical difference determined (p = 0.851). Preterm delivery was recorded in 12.5% of the anti-TG group and in 14.2% of the anti-TPO group, with requirement for neonatal intensive care unit (NICU) in 25% and 17.8%, respectively, and no statistically significant difference determined between the groups (p = 0.585 and p = 0.198). Monitoring of the maternal thyroid status and thyroid function tests during pregnancy is of great importance. Pregnancies complicated by thyroid autoantibody positivity require more care in terms of possible maternal–fetal neonatal adverse pregnancy outcomes.
