Özet

Global aphasia is called to the language disorders of which all the modalities of language are affected heavily. Lesions causing global aphasia generally enclose all the perisilvian area. Consequently both Broca area and Wernicke area and also arcuat fasciculus are included. Rarely lesions of deep subcortical structures may cause global aphasia. We are introducing a right handed 70-year old case presenting with right-sided weakness and inability to speak, and had ischemic infarct of the dominant subcortical areas, shown by brain computed tomography (CT) and cranial magnetic resonance (MR) investigations. The possible mechanism of occurrence will be discussed. A possible mechanism may be diachisis and extend to cortical areas functionally related to the lesion areas. The open collateral vascularization of the cerebral cortex may be the reason of non-occurrence of infarct in this patient.
