Objective: Metformin, one of the most prescribed drug used in diabetes mellitus (DM) and prediabetic patients, can lead to vitamin B12 malabsorbtion which is poorly recognized by the majority of physicians. Its known that total serum vitamin B12 is a late, relatively insensitive and unspecific biomarker of deficiency compared to holotranscobalamin (holoTC). In our study we aimed to clear out the vitamin B12 status of type 2 DM and prediabetic patients after treatment with metformin for 3 months by measuring serum total vitamin B12, homocysteine and holoTC levels Material and Method: A total of 30 newly diagnosed DM (n=17) and prediabetes (n=13) patients received metformin. In the beginning and after 3 months of therapy, total serum vitamin B12, homocysteine and holoTC measurements were performed. Results: After 3 months of metformin therapy, subjects demonstrated a significant decrease in serum total vitamin B12 and holoTC. There was a significant increase in serum homocysteine levels. After the treatment with metformin, according to serum total vitamin B12 levels 2 of the patients (6.6%); and according to serum holoTC 16 of the patients (53.3%) had vitamin B12 deficiency. Conclusions: Compared to serum total vitamin B12, holoTC measurement is more sensitive and specific biomarker of vitamin B12 deficiency which is seen more than we thought after metformin treatment.
