Objective: Investigation of differences in the remission and recurrence rates of microadenoma and macroadenoma patients with Cushing's disease following transsphenoidal surgery (TS). Material and Method: Records were reviewed for 31 microadenoma and 9 macroadenoma patients who had undergone TS for Cushing's disease over a 12 year period and their remission and reccurence rates were compared. Results: Remission was achieved in 26 (83.9%) of 31 microdenoma patients following TS. 3 (11.5%) of these 26 patients developed recurrent disease. One of them achieved remission following a second TS, the other underwent adrenalectomy and last one didn't came to outpatient clinic visit after recurrence. Adrenalectomy was performed for 2 of the 5 microadenoma patients with persistant disease, other 3 patients underwent a second TS. Two of them achieved remission. Adrenalectomy and radiotherapy was performed for the patient who had persistent disease. Remission was achieved in 8 (88.8%) of 9 macrodenoma patients following initial TS and 1 patient achieved remission after second TS. Two (25%) of the macroadenoma patients who had recurrent disease, had radiotherapy after adrenalectomy. The mean duration of follow up for patients who remained in remission was 43.6±51.1 months in microadenoma and 24.7±11.9 months in macroadenoma patients. The mean time to recurrence was 28.7±10.6 months in microadenoma and 37.0±15.6 months in macroadenoma patients. Conclusion: We could not find significant differences between microadenoma and macroadenoma patients in achieving remis sion, the mean duration of follow up for patients who remained in remission and reccurence rates in patients with Cushing's disease following TS.
