Clinicopathological evaluation of intra-abdominal cysts managed by surgeons in an endemic region

Background: Intra-abdominal cysts (IC) may affect several organs in the abdomen and thus represents a major problem for the general surgeon. Patients and Methods: The aim of this study was to analyze the multiple clinical presentations of the IC and the surgical options according to the involved organ. The medical records of 89 patients operated on for IC in our department during the period 1997-2007 were retrospectively reviewed. Results: The most frequent complaints of the patients were abdominal pain, nausea-vomiting, fatigue and weight loss in 84 (79.7%), 46 (51.6%), 17(19.1%) and 5(5.6%) patients, respectively. Jaundice was found in 8(8.9%) cases. Preoperative diagnoses were made by US, and in suspected cases, the diagnoses were confirmed with CT and MRI. The most commonly involved organ was the liver (49 patients; 55%), followed by the abdominal cavity (16 patients; 17.9%) and the ovary (12 patients; 13.4%). Hydatid cystic disease was found in the liver, the abdominal cavity, the spleen and the pancreas in 47 (52.8%), 3(3.3%), 2(2.2%) and 1(1.1%) patients, respectively. Simple cysts were found frequently in the over (22.4%) and the abdominal cavity (4.4%). The surgical procedures performed included cystotomy, drainage, with omentoplasty (for liver cysts), cyst excision or excision of the involved organ. Postoperative morbidity was found in 7.8%. Conclusion: Hydatid disease should be included in the differential diagnosis of cystic masses in the abdomen, especially in endemic regions.
