The Analysis of Earthquake Victims with Thoracic Trauma: Kahramanmaraş Earthquake

Özet

Two earthquakes occurred, nine hours apart, in Türkiye on February 6th, 2023, with magnitudes of 7.7 megawatts (Mw) and 7.6 Mw on the Richter scale. In this study it is aimed to analyse the demographic characteristics and treatment processes of 228 patients evaluated by the thoracic surgery clinic. In this study, the demographic characteristics of the patients, the time spent under the rubbles, the transportation time and treatment processes have been evaluated. The average age of the patients is 47.68 SD19.08 years. Of the patients, 108 (47.4%) were man and 120 (52.6%) were woman. The median time spent under the rubbles of 124 patients is eight hours (min: 1-max: 132). Pneumothorax has been detected in 30 (13.1%) of the patients, contusion in 44 (19.3%) of the patients, and rib fractures in 102 (44.7%) of the patients. Mortality has occurred in 13 (5.7%) of the patients. It is determined that the rate of tube thoracostomy (TT) and pleural catheter (PC) application to patients admitted to intensive care (37.2%) is significantly higher than the rate of TT and PC application to patients not admitted to intensive care (7%) (p < 0.001). A statistically significant difference has been found between the rate of patients who underwent TT and PC and developed mortality (46.2%), and the rate of patients who underwent TT and PC and did not develop mortality (p = 0.03). Also, the intensive care unit stay of patients who underwent TT and PC is found to be significantly higher at eight days compared to those who did not (p < 0.001). It is vital to detect thoracic trauma in the initial evaluation of patients in the earthquake zone. Performing surgical procedures in thoracic trauma affects admission to intensive care units and mortality.
