Underwater sports are performed either with an aqualung or free-diving. Today, the attraction to these sports is increasing. The complication rate and the case number are increasing because of this attraction. The well known among these complications are pulmonary air embolism, pulmonary haemorrhagia and related complications. These complications are very lethal that patients can easily lose their lives. There are some other clinical scenarios, which can be observed and concluded as nonsignificant. Epistaxis after barotrauma to paranasal sinuses, vertigo, cephalgia and ear membrane perforation are the main complications. Pulmonary barotraumas which have been described in breathhold divers are linked to environmental pressure increase. The increase in environmental pressure after underwater diving that causes total lung volume to come closer to residual volume and changes the total pulmonary capacity are reported among underwater divers. Intrathoracic negative pressure increases after deep inspiration and following voluntary diaphragmatic contraction, which the divers aim to stay longer underwater during free-diving. Pulmonary capillary pressure increase may damage pulmonary capillaries leading to pulmonary hemorrhagia. In this case report, we tried to point the importance of pulmonary hemorrhagia by presenting a patient with hemoptysis resulted during breath-holding free-diving which was finally diagnosed as pulmonary hemorrhagia.
