The place of topical corticosteroids in dermatology

Özet
The immune system plays an important role in the pathogenesis of many dermatological diseases. Therefore, agents that affect immunological mechanisms are frequently preferred in the treatment of dermatological diseases. Depending on the disease being treated, sometimes immunosuppression and sometimes immunstimulation may be required. Glucocorticoids are vital hormones for the human organism which are released in physiological doses by the adrenal cortex. Corticosteroids, which are synthetically produced analogues of glucocorticoids are frequently prescribed drugs in dermatology as well as in the other areas of medicine. In addition to its immunosuppressive and immunomodulatory effects, it also carries the anti-inflammatory, antiproliferative, anti¬allergic properties and therefore can be used in the treatment of many dermatological diseases with different pathogenesis. Corticosteroids can be used systemically or locally depending on the type and severity of the disease. However, corticosteroids do not have a target-specific effect. Therefore, especially in long-term use, it may cause side effects involving many organ systems. For this reason, it is important to know the side effects of corticosteroids, which may occur in both local and systemic use, and adjust the treatment method, dose and duration accordingly. In the treatment of dermatological diseases, it is very important to choose the most effective drug and administration way of it and to minimize their possible side effects as much as possible. The hypothalamic-pituitary-adrenal axis (HPA) should be well known for their systemic use and it is necessary to have knowledge about the potency of corticosteroids and local duration of action for local (topical and intralesional) use. In this section an overview of the systemic and topical corticosteroids used in dermatology will be mentioned and new classification methods that are developed to improve efficacy and minimize side effects of them will be discussed.
