Özet

OBJECTIVE: We studied the incidence and types of genital human papillomavirus infections and the efficacy of trichloroacetic acid. STUDY DESIGN: Colposcopic, cytologic and histologic examinations were performed on 342 patients in this randomized, longitudinal study, HPV-DNA types 6/11, 16/18 and 31/33/51 were studied with in situ hybridisation method on 42 patients who had genital HPV infections. Eighteen patients were included in the control group and TCA was applied 3 times a week for 4 weeks on 24 patients. Colposcopic, cytologic and histologic follow-up evaluations were done on 4th and 16th weeks to determine the cure rates and recurrences. Mann-Whitney U and Fisher's exact tests were used for statistical analysis. RESULTS: We found 43 genital HPV infections in the studied 342 patients (12.6 %). The cure rates after 4 weeks-follow-up were 18/24 (75 %) in TCA group and 3/18 (16.7 %) in the control group (p<0.05). Sixteen weeks follow-up revealed 5 recurrences (27.8 %) in the TCA group and 1 recurrence (33.3 %) in the control group. HPV-DNA type 6/11 was found in 21 patients (48.8 %), type 16/18 in 8 (18.6 %), type 31/33/51 in 2 (4.6 %), type 6/11 and 16/18 together in 3 (7 %) of patients, respectively. None of 3 types was found in 9 patients (20.9 %). HPV infection was found on 42 of 296 patients who do not use condom (14.2 %) and only in one of 42 patients who use it (2.2 %) (p<0.05). CONCLUSION: The incidence of HPV infection is 12.6 % and infection is mostly found in ages between 20 and 24 and HPV-DNA 6/11 are the most common types. TCA is an effective therapeutic procedure and recurrence rates are not higher from the other procedures and also condom prevents disease transmission. Infection is mostly seen in nulliparas, non-marrieds having intercourse and Class-II Pap smears.
