Özet

Some treatments for diseases have been thought to produce large effects on death or disability. Such expectations might derive from extrapolation of the effects of treatment on surrogate outcomes. For example, cardiac arrhythmias are associated with poor prognosis, and antiarrhythmic drugs can markedly reduce their frequency. However, various antiarrhythmic regimens have been found to increase, rather than decrease mortality. So, trials of new therapies in medicine should typically compare the new treatment to a control group, The control group receives the treatment against which the test intervention is being compared. These trials as referred as controlled trials. Randomized controlled trials typically involve the randomization of patients to either the control or test treatments. Randomization has important constructive influences on selection bias or likelihood of comparable groups, These trials are the gold standard for evaluating new therapies, and they form the foundation for the highest levels of recommendation in practice guideline documents that stress evidence based medicine. Although such trials provide valid comparison of therapies, the generalization of results must be analyzed cautiously. In this article we discussed some possible drawbacks of randomized-controlled trials. Clinicians should be aware of the drawbacks of randomized controlled trials during the interpretation of results.
