Ulcerative colitis in the elderly in Turkey

Several previous report regarding the clinical behavior and course of ulcerative colitis in the elderly were conflicting. We therefore undertook this study to determine the clinical characteristics of the disease in patients in whom the onset of symptoms and diagnosis occurred after age 60. The patients who were followed up with regular intervals and aged between 60 and 81 were reviewed and compared with a younger group. Some variables encompassing clinical, laboratory and medical and surgical aspects of treatment were analyzed. Forty-one patients (11.2%) had the onset of symptoms and diagnosis made after age 60. Sex prevalence, the extend of the disease, the laboratory data in older patients were similar to those observed in younger patients. Older patients have more severe initial attack than the younger patients, but clinical remission was obtained in most of the older patients and only 2.4% required surgery. In contrast, in the younger patients refractory disease (10.1%) and surgery requirement (7.6%) was more frequent. Inflammatory bowel disease is not uncommon in the elderly. Although initial attack is more severe in these patients, clinical remission is obtained with prompt diagnosis and proper therapy.
