Objective: To determine the tendon involvements and enthesal abnormalities in patients with rheumatoid arthritis (RA) using high-resolution ultrasonographic images, to compare the findings with those seen in healthy controls, to compare the ultrasonographic findings with clinical findings, and to scoring the findings observing lower limb enthesal sites in RA patients. Methods: 20 patients with RA (19 females, 1 male) and 10 healthy controls (9 females, 1 male) matched by age and body mass index (BMI) were included in this study. All the patients and controls underwent clinical and ultrasonographic examination of both lower limbs at five enthesal sites (superior and inferior pole of patella, tibial tuberosity, Achilles tendon, plantar aponeurosis) and both upper limbs for two tendons (tendons of m. biceps brachi and supraspinatus). Glasgow ultrasound enthesitis scoring system (GUESS) was used for the analysis of the involvements of enthesal sites in lower limbs in RA patients. Results: Mean age of the 20 patients with RA was 45.70 ± 7.65 years and 10 healthy controls was 42.60 ± 10.51 years. On clinical examination 54/280 (19%) enthesal sites were abnormal and on ultrasonographic examination 118/280 (42%) sites were abnormal. As compared to healthy controls, RA patients had tendinitis (tendon thickened) in all examined sites except for plantar aponeurosis. Lower limb enthesal abnormalities were found to be more in the RA patients than controls. Statistically significant correlation was found between the Ritchie articular index and GUESS. Conclusions: Tendon involvements and enthesal abnormalities in RA patients were found more than estimated.
