Özet

Objective: We aimed to assess the effect of high dose iodine (I)-131 on the outcome of pregnancies and babies of the mothers who had been treated with I-131 for differentiated thyroid cancer. Material and Methods: Forty-one female patients who had been treated with high doses of 1-131 for differentiated thyroid cancer and subsequently became pregnant were evaluated. Ages of patients at initial therapy varied from 16 to 38 years (mean ± SD= 29.1 ± 5.5 years) and the interval between I-131 therapy and conception varied between 0.3-95 months (mean ± SD= 30.09 ± 22.7). Forty-one patients were treated 49 times with radioactive iodine and the total I-131 dose for each patient varied from 3.7-7.4 GBq (mean ± SD 4.536 ± 1.524 GBq). All patients were treated with L-thyroxine at doses sufficient of suppressing thyroid stimulating hormone. Forty babies of these mothers were followed-up and six pregnancies are still in progress. Results: Two patients had spontaneous abortions, one of them during first trimester and the other in the second trimester and two patients had stillbirths. One baby was hypothyroid at birth but had no any other abnormality. Rest of the children showed normal motor and mental maturation and no one had any detectable congenital abnormality. In all of them serum TSH and thyroid hormone levels were in normal range and no one has clinically observed diffuse or noduler goiter. Conclusion: Administration of I-131 seems to have no adverse effect on the outcome of subsequent pregnancies and it is not necessary to dissuade young female patients who are treated with 131-I, from considering pregnancy. But it would be wise to advise the patients to avoid pregnancy at least 6 months after I-131 administration. Copyright © 2010 by Türkiye Klinikleri.
