Özet

We aimed to diagnose the effects of isoflurane and enflurane on carbohydrate metabolism and the stress response of the organism. We scheduled to calculate the level of insulin and C-peptide for the evaluation of the function of β cells of the pancreas and level of cortisol for the stress response. 40 male patients of ASA I undergoing herniography were included in the study. They were separated into two groups 20 patients in each. During induction, 2-3 % MAC isoflurane in 3 L O2 and 3 L N2O in group I and % 2-3 MAC enflurane in 3 L O2 and 3 L N2O in group II were delivered, respectively. Any other intravenous agent wasn't used during induction. After the eyelash reflex was abolished, tracheal intubation was performed following the delivery of 0.1 mg/kg of vecuronium bromide. Blood samples were taken before the induction, at 30th and 60th minutes following induction and serum cortisol, insulin and C-peptide levels ware measured. Glycemia was measured before the induction, and the measurements were repeated at 30th, 45th and the 120th minutes after the induction. The results were evaluated by Student's t test. In both of the two groups, significant increase in the levels of glycemia and cortisol and decrease in insulin and C-peptide were found regarding to the control values (p<0.05). In the corresponding comparison of the two groups, no significant change in any parameter was found (p<0.05). As a result, it was concluded that none of these two inhalational agents were better than the other one. In the suspicious cases having no potential to compensate increases in glycemia, additional sedative and analgesic drugs are needed.
