Özet
Objective: To evaluate whether low levels of first trimester PAPP-A are related to increased rate of fetal distress during labor and cesarean section delivery. Material and Methods: Between 01 January 2007 and 31 December 2008, 3333 patients admitted for first trimester screening to Perinatology Department were recruited in this study. Solid phase chemiluminescent immunometric assay technique. The number of patients who had a PAPP-A below 0.5 MoM was 1601 while the remaining 1732 had a PAPP-A level above this cut off. Pregnancy outcomes of the two groups were evaluated for intrapartum fetal distress and need for cesarean delivery. Results: Age and weight of the patients in both groups were similar. However; smoking was more frequent in the control group (p=0.014). Gravida was higher and fetal birth weight was lower in the study group (p=0.008 and p<0.001 respectively). Among the patients with PAPP-A level<0.5 MoM 116 (7.24%) out of 1601 developed intrapartum fetal distress and ceserean delivery was executed while 103 (5.94%) out of 1732 patients with a PAPP-A level>0.5 MoM underwent cesarean delivery due to intrapartum fetal distress. But the difference between the groups was not statistically significant [OR: 1.235; confidence interval (CI): 0.939-1.626, p=0.131]. Conclusion: Although it was not statistically significant, the rate of cesarean delivery due to intrapartum fetal distress was higher in the group with PAPP-A<5 MoM.
