Combined Endarterectomy-Interposition Technique Using Specially Shaped Polytetrafluoroethylene Graft in the Surgical Treatment of Carotid Artery Stenosis

Özet

Background: The surgical management of carotid stenosis is effectively performed using classical and eversion endarterectomy techniques. However, a range of early and late complications, most notably perioperative stroke, can occasionally arise. We applied a novel technique in our patients, with the aim of lowering of complications associated with endarterectomy. Methods: We applied this technique to 14 patients, 12 male and 2 female, with an average age of 68.26 ± 12.1 years (range 56–83). The technique is essentially based on reshaping a polytetrafluoroethylene (PTFE) tubular graft that its size appropriate to the diameter of the internal carotid artery. This shape is created by leaving a portion of the PTFE tubular graft tubular and cutting a portion of approximately equal length into a patch. This way, the tubular portion of the single-piece PTFE graft replaces the diseased proximal part of the internal carotid artery, while the patch-shaped portion is used for patch angioplasty on the common carotid artery. Results: No thromboembolic neurological complications occurred in any patient during the perioperative period, and no revisions due to bleeding were required. One patient developed temporary dysphagia. All patients were asymptomatic during the 12-month follow-up period, and follow-up imaging revealed no stenosis requiring reoperation in any of the cases. Conclusion: We believe that this technique has the potential to improve the outcome of carotid endarterectomy. If its effectiveness is confirmed through more widespread use, it may become the preferred technique for the treatment of carotid artery stenosis.
