The comparison of the effects of isoflurane and halothane on blood sugar level in the diabetic patients

Diabetics are risk patients for the anesthetists not only because they require special metabolic management and often have other concurrent diseases, but also because of interactions between anesthetics and insulin. Stress, such as hypoxia, anxiety or operations increase circulating catecholamines and this elevates blood sugar. All these factors make a diabetic patient risky in anesthesia practice. The aim of this investigation was to compare the effects of isoflurane and halothane on the blood sugar. Fourty diabetic patients ranging in age from 27-72 years undergoing upper abdominal elective surgical procedures were anesthetized with isoflurane (n = 20) or halothane (n = 20) in combination with concentrations of nitrous oxide 70% and oxygen 30%. All were premedicated with 10 mg diazepam orally. The average age, sex distribution and the type and the duration of the operations were not significantly different in the groups studied. Anesthesia was induced with thiopenthal. Blood samples were taken for blood sugar level determination. The first sample was taken before the premedication, the second, the third and the fourth samples were taken 15 min., 30 min. and 60 minutes after the induction of anesthesia. The last blood sample was taken in the recovery room. During anesthesia the usual symptoms of hypoglycemia and hyperglycemia were absent. In 10 minutes and 60 minutes after the induction an increase in blood sugar level were seen in two groups. The lower levels were obtained in patients anesthetized with isoflurane. In conclusion, isoflurane makes the least alteration in blood sugar level compared with halothane and should be prefered in diabetic patients.
