Percutaneous transhepatic biliary drainage was applied to 88 patients with obstructive jaundice mainly for obtaining palliation. All interventions were performed under fluoroscopic and/or ultrasonographic guidance. Ultrasonography and/or computed tomography were used for follow-up. Sixty six patients who had a follow-up time between one day and 23 months died because of their primary disease or the reasons related with it. Twenty two patients are still under follow-up. The average life time after the procedure was 5 months. The short-time complication rate of the procedure was 9.06% and the long-term complication rate was 30.42%. The most common short-term complication was non-effective drainage and long-term complication was catheter care problems such as dislocation and breakage. Cholangitis or symptoms related to cholangitis regressed in all patients except one. Hyperbilirubinemia and related symptoms were eliminated in all patients. Surgery was undertaken for 12 patients after obtaining the convenient conditions by biliary drainage. Endoprotheses were applied in 28 patients with inoperable malignancy. External or external-internal biliary drainage was performed in the rest of the patients. The results of the present study reveal that percutaneous transhepatic biliary drainage is a safe and effective procedure for relieving the obstructive manifestations.
