Complications and treatment of brucellosis: 11-year results

Introduction: Brucellosis is a preventable infectious disease in low- and middle-income countries (LMICs), including Turkey, but remains a significant public health problem. Our study aimed to examine the complications of brucellosis which developed over an 11-year period. Material and methods: We evaluated 220 cases, followed up and treated between January 2001 and December 2012, with data obtained from patient files in the clinic archive. Results: The most commonly involved system was the musculoskeletal (47.3%), followed by haematological (46.4%), gastrointestinal (35%), neurological (5.5%), genital (4.5%), dermatological (1.8%), and cardiovascular (1.4%). Capillary leak syndrome developed in one patient, being recorded for the first time. In uncomplicated cases, a combination of treatment with doxycycline and rifampicin was effective. Drugs including aminoglycosides, trimethoprim-sulfamethoxazole, and quinolone were added to this combination where complications ensued; duration of their treatment was inevitably lengthened. Conclusion: Brucellosis severely impinges on a counntry’s economy by affecting not only human health but also animal husbandry. Because of difficulties experienced in diagnosis and treatment in endemic areas, large-scale studies are needed to create protocols to recommend appropriate treatment options particularly for complications and during follow-up.
