Özet
The aim of this study was to compare the analgesic efficacy of caudally administered tramadol with bupivacaine in children. We enrolled 30 paediatric patients undergoing lower abdominal surgery between the ages of 12-60 months in a prospective study. None of these children received premedication. They received randomly caudal block with 0.8 mL/kg of 0.25 % bupivacaine in group I and 0.8 mL/kg of 2 mg/kg tramadol in group II. Caudal block was performed under sevoflurane anaesthesia during which no other opioid analgesic was used. Peroperatively mean arterial pressure (MAP), heart rate (HR) and oxygen saturation (SpO2) was obtained 5 min. before the induction of anaesthesia and every 5 min. thereafter. Postoperatively MAP, HR, Sp02, sedation scores, analgesia scores (with modified TPPPS) and respiratory rate were recorded every hour until 6 hr. after injection. After discharge, the patients were assessed for sedation and analgesia every hour for another 6 hr at home by phone. There were no statistically significant difference regarding sedation scores and analgesia (p>0.05). Duration of analgesia was longer (9.1 h) and nausea and sweating was observed significantly more in tramadol group (10<0.05). Tramadol provides an effective and safe analgesia through caudal administration in children undergoing lower abdominal surgery.
