The association between a high procalcitonin level and mortality in palliative care patients: A retrospective cohort study

Background and aim: Procalcitonin (PCT) is a strong prognostic biomarker in bacterial infections. There are few data on the role of PCT in the prognosis of palliative care patients. The aim of this study was to determine if the serum PCT concentration in palliative care patients is associated with mortality. Material and methods: A retrospective cohort study was performed between September 2013 and February 2014. The study included 100 consecutive critically ill patients that were admitted to the ICU at Ulus State Hospital. Within 24 h (hours) of admission to the ICU PCT, C-reactive protein (CRP), albumin, the white blood cell count (WBC), haemoglobin, lactate dehydrogenase (LDH), lactate, and Acute Physiology And Chronic Health Evaluation (APACHE II) scores were measured. Results: ROC-AUC curves for both the PCT concentration and APACHE II score were significantly associated with prognosis (P < 0.001). An APACHE II cut-off score of ≥21.5 and a PCT cut-off concentration of ≥0.395 mg dL-1 were selected based on the maximum sensitivity and specificity obtained from ROC curves. The PCT concentration might be a predictor of mortality in palliative care patients, because the ROC-AUC with an asymptotic 95% confidence interval (CI) was 0.626-0.826 and asymptotic P < 0.001. The log-rank test (P = 0.001) indicated that the PCT concentration is a reliable indicator of mortality. Conclusions: In the present study there was an association between the PCT concentration and mortality in palliative care patients. Whether the PCT concentration is confirmed to be a strong indicator of mortality, its measurement could contribute to effective palliative care planning.
