Electrocardiographic (ECG) abnormalities are commonly seen with tumor invasion of the heart and pericardium. Most of these abnormalities are nonspecific, but certain findings may be highly suggestive of cardiac involvement including ST segment elevation, T wave abnormalities, and Q waves in the absence of infarction. A patient with known malignant pleural mesothelioma referred to the hospital because of chest pain. An ECG showed ST segment elevation in the inferior and anterolateral leads. Non-invasive cardiac studies disclosed invasion of the heart and the pericardium by the tumor. ST segment elevation without evolutionary ECG changes and the development of pathological Q waves may be important signs of pericardial invasion in cancer patients.
