Correlation between pulse pressure and urinary albumin excretion in type 2 diabetic patients without microalbuminuria

BACKGROUND: Urinary albumin excretion is known to be associated with hypertension in type 2 diabetic patients. In many studies, a strong relation has been found between urinary albumin excretion and pulse pressure In this study, we aimed to investigate the relationship between urinary albumin excretion and pulse pressure in type 2 diabetic patients without microalbuminuria. METHODS: Nonmicroalbuminuric type 2 diabetic patients were examined retrospectively. Demografic, laboratory data, urinary albumin excretion, blood pressure and pulse pressure measurements were obtained from whole patients. RESULTS: A total of 588 patients (70.1%, female) were included in the study. The mean age was found to be 57.2±9.7 years. It was found that 37.4% (N.=220) of the patients were in the group with a PP of ≤46 mmHg (group 1), 40.8% (N.=240) were in the group with a PP of 46-56 mmHg (group 2) and 21.8% (N.=128) were in the group with a PP of ≥56 mmHg (group 3). There was a significant correlation (r=0.458) between pulse pressure and urinary albumin excretion independent of systolic, diastolic, or mean blood pressure levels among nonmicroalbuminuric patients with type 2 diabetes mellitus. CONCLUSIONS: Pulse pressure may be a sign of progression to microalbuminuria. Increased pulse pressure may be used as a risk factor for development of diabetic nephropathy.
