Özet
Aim: We aimed to compare the analgesic efficacy and side effects of intrathecal (i.t) neostigmine and i.t fentanyl added to hyperbaric bupivacaine for the relief of pain and postoperative nausea and vomiting (PONV) in transurethral resection of prostate (TUR-P). Material and Methods: This prospective, double blinded, randomized study was performed in 60 ASA I-II patients undergoing TUR-P. Patients assigned to recieve 2 mL i.t 5% hyperbaric bupivacaine with 1ml of either normal saline, neostigmine 25 μg or fentanyl 25 μg The onset of anesthesia, maximal level of sensory block, sensory regression to S1-2 dermatome, bromage score, use and timing of rescue analgesic and antiemetik, visual analog pain scores during the first 24 postoperative hours and the incidence of side effects were recorded. Results: Bromage scores were significantly high and need for rescue antiemetic therapy higher in the neostigmine group. Side effects were similar between the fentanyl and neostigmin groups exception of nausea and vomiting occurred in the neostigmin group more frequently than in the control and fentanyl groups. Conclusion: We conclude that intrathecal neostigmine, or fentanyl combined with hiyperbaric bupivacaine provides effective analgesia for TUR-P. However, nausea or vomiting associated with neostigmine administration limits clinical use.
