Özet

There are contradictory reports about the effects of rHu EPO on cardiac functions and some vasoactive hormones in patients with renal anemia. To obtain a clearer knowledge, we studied 17 anemic normotensive patients undergoing chronic hemodialysis and 10 healthy volunteers. Patients received 72 ± 23 IU/kg. body weight of rHuEPO intravenously thrice weekly after dialysis for three months. Cardiac functions were determined by echocardiography in patients and controls. After treatment, hemoglobin levels increased, and mean arterial pressures did not alter. High cardiac output, ejection fraction and fractional shortening declined to normal values. The end-systolic pressure/end-systolic volume ratios which represent contractility function of the left ventricle did not change. Atrial natriuretic peptide (ANP) levels which were high before treatment did not alter after treatment, but high renin and aldosterone levels decreased to normal levels. As a result, after rHuEPO treatment, anemia improved and therefore so did hyperkinetic cardiac functions.
