Photosensitive epilepsy: Triggering factors, diagnosis and treatment

Photosensitive epilepsy (PE) is the most seen type of reflex epilepsy (RE) group, characterized with seizures triggered by sudden and rapid light stimuli. Photoparoxysmal responses (PPRs) can be seen during intermittant photic stimulation (IPS). Avoidance of triggering factors and, if needed, additional antiepileptic drug treatment is mostly useful for the seizure prevention. Valproic acid (VPA) is the most used drug for this. We evaluated 14 patients with PE with their family history, electroencephalography findings, seizure types, triggering factors, seizure frequency before and after treatment, and treatment response. Four of them had positive family history. Photoparoxysmal responses were seen generally during 15,20 and 25 Hz. frequency of photic stimulation. Ten of them had generalized tonic-clonic (GTC), 3 of them had myoclonic and one of them had both GTC and myoclonic seizures. Thirteen patients were treated with VPA and one of them with Lamotrigine (LTG). 2 patients treated with VPA did not respond to therapy because of not avoiding of triggering factors or not using the antiepileptic drug properly. Our results suggest that PE has a good prognosis. Seizure frequency before the treatment is not related with outcome. Avoiding of the triggering factors must be the main step of the treatment. If an antiepileptic drug needed, first drug of choice is VPA. © Universitätsverlag Ulm GmbH 2006.
