Özet

Hypertension is one of the major causes of morbidity and mortality, yet, especially in middle and advanced age population, although great improvements gained in the treatment modalities. The goal of this study is to describe hypertensive conditions and acute complications, on the basis of patient characteristics. In this study, we reviewed the annual records of the Emergency Department, and included the patients at age of 40 and over (n=8226). The data of patients', who were diagnosed as (1) hypertension (BP:140/90 mmHg-over), (2) and related complications (neurologic, cardiac etc.), (3) hypertensive emergencies (BP:240/160 mmHg-over) (total n=986) collected and than analyzed in respect of patients' characteristics, recent antihypertensive medications, executed treatments in ED and outcomes. The majority of the cases were women (66%) and the mean age was 64.5±12 years. The mean arterial BP was 180±30 / 112±20 mmHg (sistolic/diastolik, mean: 127±8 mmHg), and there were no significant age and gender differences. Significant hypertension was detected in 674 patients (160/110 mmHg-over, 68%) and 30 cases were hypertensive emergency (240/160 mmHg-over and/or systemic complications, 3%). The most frequent complications were related with cardiovascular and neurologic systems. ACEI (63%) and diuretics (15.8%) were the preferred urgent treatment options, and 148 of patients (14.9%) were hospitalized which were unresponsive to treatment or required further interventions. At the admittance, questioning of patients' recent medication for hypertension revealed; ACEI 19%, combinations-in-one 19% (namely ACEI or ARB plus diuretics), calcium blockers 14% and nitrates 12% were the most frequent. Hypertension was diagnosed as the first time only in 71 of patients (7%). At the result of the analysis, we detected that merely 156 of patients (17%) were taking their medication in regular, 421 patients not regularly (46%), and in 338 had never used their prescribed medications (37%).
