Özet
Purpose: The frequency and popularity of immediate sequential bilateral cataract surgery (ISBCS) is increasing. We discuss the course of rehabilitation for a patient who underwent ISBCS and developed toxic anterior segment syndrome (TASS) in both eyes. Methods: This study is a case report. Results: The patient was a 42-year-old woman with no remarkable past medical history who underwent ISBCS at another hospital in 2023. Intraoperatively, mitomycin C (MMC) was inadvertently instilled into the anterior chamber of both eyes at the conclusion of surgery instead of carbachol (Miostat®). The next day, the patient noticed vision loss, and an ophthalmological examination revealed corneal edema in both eyes. She was referred to our hospital with the diagnosis of bilateral TASS. Although the left eye began to improve after a week of medical treatment, the right eye finally required Descemet's membrane endothelial keratoplasty. At the 2-month postoperative visit, uncorrected distance visual acuity was 0.7 bilaterally, and both corneas were clear on slit lamp biomicroscopy. Conclusion: Although ISBCS provides convenience in many areas, we believe that it should be performed only for particular indications and conditions, because it may lead to severe problems such as bilateral blindness. Objectif: La fréquence et la popularité de la chirurgie de cataracte bilatérale séquentielle immédiate (ISBCS) augmentent. Nous discutons du parcours de réhabilitation d'un patient ayant subi une ISBCS et développé un syndrome toxique du segment antérieur (TASS) dans les deux yeux. Méthodes: Cette étude est un rapport de cas. Résultats: La patiente était une femme de 42 ans sans antécédents médicaux remarquables, qui a subi une ISBCS dans un autre hôpital en 2023. Lors de cette intervention, la mitomycine C (MMC) a été administrée par inadvertance dans la chambre antérieure au lieu du carbachol (Miostat®) à la fin de la chirurgie pour les deux yeux. Au lendemain de l'opération, elle a remarqué une perte de la vision et un examen ophtalmologique a révélé un œdème cornéen dans les deux yeux. Elle a été référée à notre hôpital avec le diagnostic de TASS bilatéral. Bien que l’œil gauche ait commencé à s'améliorer après une semaine de traitement médical, la détérioration de l’œil droit a nécessité une chirurgie de kératoplastie endothéliale de la membrane de Descemet. Lors de la visite postopératoire du 2e mois, l'acuité visuelle à distance sans correction était de 0,7 bilatéralement, et les deux cornées étaient claires à la microscopie à lampe à fente. Conclusion: Bien que l'ISBCS offre des avantages dans de nombreux domaines, nous pensons qu'elle ne devrait être réalisée que dans des indications et des conditions particulières, car elle peut entraîner des problèmes graves tels que la cécité bilatérale.
