Limb Preservation with Free Latissimus Dorsi Myocutaneous Flap in a Patient with Rothmund-Thomson Syndrome

Özet
Rothmund-Thomson syndrome (RTS) is a rare genetic disorder characterized by poikilodermatous rash, skeletal abnormalities, impaired wound healing, and an increased risk of malignancies. This case report details the successful usage of a free latissimus dorsi myocutaneous flap for limb preservation in a 40-year-old male RTS patient with a transtibial amputation and a chronic, nonhealing stump wound. Despite the patient's delayed wound healing and cachectic state, patency of vascular structures allowed for meticulous microsurgical reconstruction. The operation has not only elevated quality of life of the patient by enhancing mobility but also avoided a more proximal amputation. This case report aims to highlight the utility of free flap surgeries in syndromic patients with impaired healing, underlining the importance of preserving limb functionality. Further studies are warranted to construct a better understanding of RTS and elevate surgical approaches to improve outcomes for RTS patients with complex reconstructive needs.
