Objective: The aim of this study was to evaluate the incidence of acute esophagitis and dosimetric factors effecting the development of mild esophagitis in patients with locally advanced breast cancer who treated with adjuvant conformal radiotherapy. Material and Methods: The radiotherapy treatment plans of 213 locally advanced breast cancer patients receiving conformal radiotherapy to supraclavicular fossa and axillary nodal area between 2014-2019, were retrospectively evaluated. Total esophagus was divided into 3 segments as upper, middle and lower parts on the treatment planning tomography. Two groups were identified as patients who experienced esophagitis and patients who did not experience esophagitis. The dosi-metric data of two groups were compared. Results: Evaluation of 213 patients showed that 48 patients (22%) reported grade1 esophagitis after 15 (5-25) fraction in average. The mean dose (Dmean) of upper esophagus was significantly larger in patients who experienced esophagitis in the ratio of 37,9% compared to ones did not (p=0.007). The upper oesophageal Dmean value was found >7.9 Gy in 24/48 patients who experienced esophagitis and ≤7.9 Gy in 109/165 patients who did not experience esophagitis (OR=1.94, %95 CI=1.01-3.73, p=0.043). Conclusions: Esophagus is exposed to certain dose of radiation in patients with locally advanced breast cancer who underwent conformal radiotherapy. According to the results, the risk of radiation induced esophagitis can be diminished by limiting the dose of upper esophagus as 7.9 Gy that is neighbor to the supraclavicular area during the conformal radiotherapy planning.
