Özet
Nowadays, the high rate of nurse turnover, especially in inpatient treatment institutions, poses a serious problem for hospital management. At the same time, the COVID-19 global epidemic, which affected the whole world in 2019, caused new changes to be added to the change in the healthcare system. These changes and the fact that Turkish nurses want to migrate to different countries for many other reasons show that the nurse turnover rate will increase. The inability to find competent and adequate nurses to fill the nursing shortage and retain these nurses in the institution is another issue. When the scientific literature is examined, many suggestions are reached about attracting and keeping nurses in the institution, and one of these suggestions is to make plans to develop the careers of nurses. Sufficient career steps cannot be created to support nurses’ career progression in the clinical environment. This situation causes nurses to have to work in the same position for many years. As a result, their motivation and job satisfaction decrease, and as the process continues, they may become estranged from the profession. The clinical ladder program is a system that allows nurses working at the bedside to pursue a horizontal career without leaving the clinical area. This concept, which was put forward in the 1970s in the United States of America (USA), was later developed and implemented by Patricia Benner. Based mostly on Benner’s model, the clinical ladder method is applied in the career planning of nurses in the USA and in different countries of the world (New Zealand, United Kingdom, Korea, China, Ghana, Lebanon, Australia) and in many healthcare institutions. Apart from Benner’s model, Carper’s “Basic Knowing Patterns” model and Martha Curley’s “Critical Care Synergy” model are also used in clinical ladder programs. It is known that a clinical ladder program is implemented in a private hospital in Turkey, based on the limited information available. This review has been prepared to attract and retain competent and adequate nurses to the institution and to provide information about the clinical ladder program that provides career opportunities to nurses without leaving the bedside and to guide nurse managers when establishing the clinical laddering system in their institutions.
