Özet

Improving the biomaterials and hemocompatibility of both the conduct and components of extracorporeal membrane oxygenation/life support (ECMO/ECLS) is crucial to improving patient care. The problem can be further defined by acknowledging that cardiopulmonary support, in its current form and practice, restricts the patient's ability to recover sufficiently enough to exit the hospital in less than the currently expected timeframe. The most important ideas are those validated by prior statistical evidence pointing to a decrease in surface area, priming volume, and blood transit time in the circuit. Leading to the next steps, by decreasing the physical length of the circuit, biomaterial, pump and cannula design, contact activation is reduced, thus avoiding the body's natural inflammatory response or defensive mechanisms. The use of systemic anticoagulation is, at this point in clinical care, considered a necessity for the successful application of ECMO/ECLS in the clinical setting. This chapter provides a framework for contemporary types of equipment in ECMO/ECLS patients, optimization, and building a procedure to utmost physiologic level and future prospectives.
