The combined prophylactic administration of H1 and H2 receptor blockers in the prevention of protamine related heamodynamic effects and the relation between antihistamines-leukocytes in pulmonary vascular resistance

Özet

We studied the combined prophylactic administration of H1 and H2 receptor blockers in the prevention of protamine related haemodynamic effects of protamine which is used to neutralize the heparin in CPBS. As for PVR, a kind of mediator approach is planned with respect to the relationship between antihistamines and leukocytes. The study was performed in two groups, each of which consisting of 15 patients. In the first group the prophylaxy was made by 50 mg raniditin HCl iv after the induction and 50 mg chlorphenoksamin HCl into the prime solution of the pump during the warming period. Both of the groups received general anaesthesia with fentanyl + diazepam + pancuronium bromide and neutralized by using protamine sulphate with the same amount of heparin. Values of cardiac output, hemogramm and ionized Ca were obtained. In control group, at 1 min. after the protamine injection is finished, significantly important changes were seen in SAP, MAP and at 5 min in DAP values. Also meaningful changes were seen in the values of PVR at 1, and 5 min and in the values of leukocytes at 10 min . A meaningful negative correlation was also seen between PVR and leukocyte at 5 min. The combined prophylactic administration of H1 and H2 receptor blockers was concluded to prevent the decreases in the SAP, MAP, PAP values. It is thought that the negative correlation between the leukopenia and PVR seen during the neutralization of heparine with heparin antagonists is the result of complement activation and the Tx generation accompanying to this. As for PVR mechanisms; antihistamines were considered to prevent the leukopenia by inhibiting the complement activation in receptor site.
