An examination of near-miss events experienced by healthcare workers in the operating room

Objective: This study aimed to examine healthcare workers’ experiences with near-miss events in the operating room and to identify their frequency, causes, and reporting behaviors. Methods: This descriptive study was conducted between March and June 2023 in a training and research hospital with 232 multidisciplinary healthcare workers. Data were collected using a demographic form and a structured near-miss reporting form, analyzed descriptively, and classified based on the WHO International Classification for Patient Safety. Results: Of the participants, 58.6% encountered a near-miss event, but only 8.8% reported it. The most frequent categories were clinical processes/procedures (34.52%) and medication/IV fluids (27.38%). Common events were wrong-site surgery, patient misidentification, and near falls. Inattention (31.34%) and inexperience (15.67%) were main causes. Suggested solutions included increased caution (25.5%), training (14.77%), and better communication (8.05%). Conclusion: Near-miss events are common but underreported; promoting safety culture and non-punitive reporting is essential.
