Substance use in the early adult male with first episode psychosis

Introduction: The prevalence of substance use disorder (SUD) in subjects with first-episode psychosis (FEP) was reported between 6-62 %. The aim of this study was to strengthen our knowledge about characteristics of substance use and its effects on clinical features in these subjects. Methods: The subjects (n=111) were divided into two separate groups as FEP subjects with SUD (n=12), and FEP subjects without SUD (n=99). Groups were compared with regard to clinical and sociodemographic data such as cigarette smoking, alcohol and substance use, duration of untreated psychosis, clinical symptoms and disease severity. Results: Cannabis was the most lifetime used substance with 18 % (n=20) of all subjects. In the SUD group, mean age (18.2±6.4) and mean duration of education (6.2±2.5) were lower compared with FEP without SUD (respectively; 23.0±4.8, 8.9±3.3). Unemployment rates (91.7 %) were higher in FEP with SUD than FEP without SUD (p<0.05). In the FEP group with SUD, premorbid functioning was better (91.7 %, p=0.00) and duration of untreated psychosis was longer (13.41±10.51 days) compared with FEP group without SUD (p<0.05). Discussion and Conclusions: The subjects with FEP in this study had lower current and lifetime SUD rates compared to literature. Although the premorbid functioning of the patients with SUD was better, substance use affected negatively duration of untreated period. © Universitätsverlag Ulm GmbH 2008.
