Effect of intrapleural bupivacaine for analgesia after thoracotomy and cholecystectomy

Özet
The effect of intrapleural regional analgesia in the treatment of postcholecystectomy and postthoracotomy pain was evaluated. ASA physical status: thirty patients eligible for the study. The patients were classified randomly into two groups (thoracotomy group, n = 15; cholecystectomy group, n = 15). In thoracotomy group epidural catheter was inserted by the surgeon through the thoracic wall, before wound closure; in cholecystectomy group, epidural catheter was introduced by the anesthesiologist with Touhey needle after completion of surgery. In recovery room, when the patients reported pain, 2 mg/kg of 0.5% bupivacaine was given to all the patients via intrapleural catheter. It was injected at 6 hours intervals four times a day. Before and 1 hour after the injection, arterial pressure, heart rate, oxygen saturation, arterial blood gases, forced vital capacity, forced expiratory volume in the first second were recorded. Pain scores (visual analogue scale, Prince Henry pain scale), were recorded at regular intervals. After the injection of bupivacaine, all patients showed increases in both forced vital capacity, forced expiratory volume in the first second. There were no significant differences in arterial pressure, heart rate, oxygen saturation and arterial blood gases (p > 0.05). In conclusion, our study demonstrated that single dose intrapleural blocks are an effective method for postoperative pain after thoracotomy and cholecystectomy. There was no significant difference between the two groups.
