Özet
In this study, we compared hemodynamic stability and quality of recovery between the three different opioid anaesthetic agents used in 45 elderly women patients during hip prothesis surgery. Anaesthesia was induced with DHBP 0.25 mg/kg in all of the patients. Then fentanyl 5 μg/kg was delivered to the first group, alfentanil 30 μg/kg to the second group and remifentanil 1 μg/kg to the third group of patients. Muscle relaxation was achieved with vecuronium. Anaesthesia was maintained with fentanyl 0.5 μg/kg/min in Group 1, alfentanil 1 μg/kg/min in Group II and remifentanil 0.2 μg/kg/min in Group III with 70 % N2O:O2. Blood pressure and heart rate was measured before, during and after the induction, immediately after the intubation, just before and at 15th and 30th minutes of the surgical procedure, after surgery at 0, 15th and 30th minutes. Spontenous eye opening, reaction to verbal stimulus and extubation time was assessed. Sedation level was assessed using Stewart sedation scale at the extubation, 15th and 30th minutes after the extubation. As a result, we found that fentanyl and alfentanil were inadequate to prevent hemodynamic response during intubation and surgical stimulation, but the blood pressure and heart rate stayed fairly permanent in Group III. Recovery period was significantly faster in Group III compared to the others. In conclusion; we obtained better results with remifentanil than the other groups at hemodynamic stability and recovery period in the geriatric patients.
