Do hydroxyethyl starch solutions (HES) in various concentrations make a change in IgE, PGE2, and HRF in male patients? CESITLI KONSANTRASYONLARDAKI HIDROKSI ETIL NISASTA (HES) ERKEK OLGULARDA IgE, PGE2 VE HRF DUZEYLERINI DEGISTIRIR MI?

Özet

Colloidal solutions are used for preventing hypovolemia and shock in gunshot wounds, in accidents, in traumas, and in burns and for maintaining normovolemic hemodilution. Our study was planned to evaluate the tendency of different concentrations of HES containing high molecular weight amylopectin and produced by corn, starch to anaphylactoid reactions in male patients. 40 male cases of ASA I-II which underwent inguinal herniography under spinal anaesthesia were included in the study. Cases were divided into four groups. Group I, II, III, and IV cases were infused 500 mL of 450/0.7, 6 % HES, 500 mL of 200/0.5 10 % HES, 500 mL of 200/0.5 6 % HES and 500 mL of 0.9 % NaCl in 30 minutes, respectively. Blood samples were taken before infusion and 12 and 24 hours after the operation for detecting levels of IgE (Immunoglobulin E), PGE2 (Prostaglandin E2) and HRF (Histamine Releasing Factor). Results were evaluated with Student's-t test and nonparametric Mann-Whitney U test. 4 cases of group I and 2 cases of group II complained pain and burning around the vein used for infusion. IgE levels of group I were higher than control values (p<0.05). But these increases were within the limits of laboratory references. No significant differences were observed in the levels of PGE2 and HRF in any groups. No clinical reaction related to colloidal solution infusion was observed. As a result, although study group was small we concluded that HES solutions in proper closes has a low incidence of anaphylactoid reactions.
