A retrospective trial comparing 5-ASA plus ciprofloxacin and metronidazole with 5-ASA and corticosteroids in the treatment of active Crohn's disease

Background/aims: Bacteria in the gut lumen may play a role in the etiology and/or symptoms of Crohn's disease. The aim of this study was to evaluate retrospectively the efficacy of corticosteroids with metronidazole plus ciprofloxacin in the treatment of acute phases of Crohn's disease when combined with conventional 5-ASA treatment. Methods: The medical records of 51 patients seen at our clinic for active Crohn's disease during the period 1993-1999 were reviewed. The patients were divided into two groups according to therapy: Group 1. Taking 5-ASA plus corticosteroids. Group 2. Taking 5-ASA plus metronidazole and ciprofloxacin. Clinical remission was defined as a Harvey-Bradshaw index of three points or less; an index reduction of at least three points indicated a clinical response. Results: Nineteen of the 31 corticosteroid group patients (61.2%) and 12 of the 20 antibiotic group patients (60%) obtained clinical remission by the end of week 12 (p = NS). Clinical response occurred in two patients (6.4%) in group 1 and two patients in group 2 (p = NS). Remission was achieved in 50% of corticosteroid and 60% of antibiotic treated patients who had relapsed following resection. In patients with disease in the large and/or small intestine significant reductions in activity index scores were found in the, corticosteroid but not in the antibiotic group (p<0.05). There were no adverse events leading to treatment failure. Conclusions: Ciprofloxacin and metronidazole offer an alternative to corticosteroids in achieving clinical remission for patients with active Crohn's disease.
