The influence of isokinetic knee exercises on Q-angle and pain in patients with patellofemoral malalignment

The aim of this study was to examine the effects of isokinetic exercises on Q-angle in patients with malalignment, and to evaluate the relation between Q-angle alterations and the relief of pain in patients with patellofemoral malalignment. Twenty-one consecutive patients with the complaint of anterior knee pain who met the inclusion criteria were recruited in this study. Patients were divided into two groups according to their Q-angles; malalignment group and normal alignment group. A total of 34 knees were examined. Tested isokinetic parameters were; peak torque, total work, average power and hamstring/quadriceps ratios. Isokinetic parameters improved significantly as well as pain scores in both groups at the end of 6-week treatment period. Q-angle decreased in both groups although it was not significant in normal alignment group. In conclusion; the isokinetic exercise program used in this study increased the strength of knee muscles in both malalignment group and normal alignment group. Pain reduced significantly in both groups but it is not likely that the improvement obtained at Q-angle is the cause of pain reduction.
