Özet

Amaç: Cerrahi müdahalenin en önemli komplikasyonlarindan biri Cerrahi Alan Enfeksiyonlaridir (CAE). Bu çalişmada 2009-2014 yillari arasinda bes cerrahi ünitede karşilaşilan cerrahi alan enfeksiyonlarinin türlerinin ve izole edilen etkenlerin antibiyotik direnç oranlarinin belirlenmesi amaçlandi. Yöntem: Bu çalişmada alti yillik süre içinde (Ocak 2009 - Aralik 2014) aktif sürveyans metodu ile CDC kriterlerine göre tanimlanan cerrahi alan enfeksiyonlari değerlendirildi. Bulgular: Alti yillik sürede beş cerrahi ünitede 800 cerrahi alan enfeksiyonu tesbit edildi. Cerrahi alan enfeksiyonu nozokomiyal enfeksiyonlar içinde bazi yillarda ikinci, bazi yillarda üçüncü en sik görülen enfeksiyon idi. Hastalarin ortalama yaşi 58,18±16,86 olup %53,07'si erkekti. çalismaya dahil edilen hastalarin 87 (%10,87)'si yüzeyel CAE, 299 (%37,37)'u derin CAE ve 414 (%51,75)'ü organ bosluk CAE idi. En fazla organ boşluk CAE Genel Cerrahi Kliniği'nde tesbit edildi. Izole edilen mikroorganizmalar Escherichia coli (% 26,16), Acinetobacter spp. (%22,53), Staphylococcus spp. (%14,63), Enterococcus spp. (%11,78), Klebsiella spp. (%8,16), Pseudomonas spp. (%7,64), Enterobacter spp. (%3,36), Candida spp. (%1,68), Proteus spp. (%1,42) ve diger mikroorganizmalar (%2,59) idi. Acinetobacter spp. yüksek oranda (%82,60-%100) meropeneme ve çok ilaca dirençli yapiya sahiptir. Genislemis spektrumlu beta laktamaz (GSBL) üreten E. coli orani 2009-2014 yillarinda %46,93-%16,66 arasinda idi. Tüm Enterobacter spp. karbapenem duyarli idi. S. aureus'un %73,33'ü ve koagülaz negatif stafilokoklarin %78,26'si metisilin dirençli idi. Enterokoklarda 2011 yilinda 2, 2014 yilinda 1 vankomisin dirençli bakteriye rastlanilmiştir. Sonuç: Her ünite uygun antimikrobiyal profilaksi ve ampirik tedavi için, kendi epidemiyolojik verilerini, CAE sikliğini ve etiyolojisini tanimlamalidir. Böylece akilci ampirik antibiyotik kullanimini yayginlaştirmanin yani sira, CAE ilişkili sağlik harcamalari ile morbidite ve mortalitenin azalmasina da yardimci olunabilir. Objective: One of the most important complications of surgery is postoperative surgical site infections (SSI). In this study, it is aimed to investigate the species and antimicrobial resistance rates of the isolates in SSI in five surgical units between 2009-2014. Methods: In this study, the SSI's which were detected by active surveillance method and defined according to Centers for Disease Control and Prevention criteria (CDC) were evaluated in the five surgery units during the six-years period (January 2009-December 2014). Results: During six-years period, 800 SSI were detected in five surgery units. SSI was the second most common type in some years and the third most common type in other years of hospital-acquired infections. The mean age was 58,18±16,86 years, and 53,07% were males. Of the patients included in the study 87 (10,87%) were incisional SSI, 299 (37,37%) were deep SSI, and 414 (51,75%) were organ space SSI. The most prevalent organ space SSI was detected in General Surgical Clinic. The isolated microorganisms were Escherichia coli (26,16%), Acinetobacter spp. (22,53%), Staphylococcus spp. (14,63%), Enterococcus spp. (11,78%), Klebsiella spp. (8,16%), Pseudomonas spp. (7,64%), Enterobacter spp. (3,36%), Candida spp. (1,68%), Proteus spp. (1,42%) and other microorganisms (2,59%). Acinetobacter spp. are higly resistant to meropenem (82,60%-100%) and has multidrug resistant patern. The ratio of E. coli which produces extended spectrum beta lactamase changed between 46,93%-16,66% in 2009-2014. Enterobacter spp. was susceptible to carbapenem. Methicillin resistance was found in 73,33% of S. aureus and 78,26% of CNS strains. It was observed that of the enterecocus, in 2011 two, and in 2014 one were resistant to vancomisin. Conclusion: Every unit should define her own epidemiologic data, frequency and etiology of SSI in order to approach their antimicrobial prophylaxis and empric treatment rationally. It may help to organize proper antibiotic use, minimize morbidity, mortality and healthcare costs associated with SSI.
