Özet

Renal transplantation from a living donor has better results than cadaveric renal transplantation in terms of patient and graft survival rate, quality of life, and cost. Renal transplantation from living donors is performed safely with positive results due to insufficient cadaveric renal supply. The most classic and commonly used donor nephrectomy technique is the open technique performed with a flank lumbotomy incision. Today, this technique has been rapidly replaced by minimally invasive laparoscopic or robot-assisted donor nephrectomy techniques. This study aims to define the surgical techniques used in living donor nephrectomy, discuss the use and reliability of these techniques in different patient groups, and examine the long-term follow-up results of donors and recipients. There was no significant difference in complication rates, cost-effectiveness, and graft function in patients undergoing laparoscopic donor nephrectomy compared to open donor nephrectomy. On the other hand, renal transplantation centers should offer donors a few different techniques according to their needs and unique circumstances. Laparoscopic donor nephrectomy has become the gold standard technique for suitable living kidney donors in the last decade. However, it should be kept in mind that open, retroperitoneoscopic, robotic, and hand-assisted techniques can be safely applied for donor nephrectomy in the presence of donor-specific risk or limiting factors such as a history of previous abdominal surgery, multiple kidney arteries or veins, or obesity.
